Online Patient Registration

You may pre register with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

• Wear comfortable clothing such as sweat pants and t-shirts on the day of your procedure. Wear glasses instead of contacts.
• Pull your hair back so that it stays away from your face while you are under. Remove any removable jewelry. No need for a lot of makeup and lipstick either.
• Bring a parent or other responsible adult to drive you home. They must stay in the office for the duration of your procedure. All patients under the age of 18 must be accompanied by a parent or guardian.
• And finally, when you are brought in from the waiting room, turn your phone off and put it away or give it to whoever came with you. Relax and know you are in good, qualified hands here at Front Range Oral Surgery’s Just Wisdom Teeth.
